We are Wellbeing Therapy Solutions based at Bodie’s House, 254 Braunstone Lane, Leicester, LE3 3AS

The General Data Protection Regulation (GDPR) is concerned with the personal information about you that we collect, store, and share. This page details our GDPR policy. Every Associate of Wellbeing Therapy Solutions is registered with the Information Commissioner’s Office

Personal Information we will collect through our online Contact Form

  • Name
  • Email address (plus permission to send you an email)
  • Telephone/mobile number
  • Data submitted in the message box
  • Services you are interested in
  • Relevant school/college/organisation

 

Personal Information we will collect through our New Client Information form on Cliniko

  • Name
  • Date of Birth
  • Address
  • Telephone/mobile number
  • Email address
  • School Year and School Name (if applicable)
  • Medical History, details of medications and your GP Surgery name

 

How we will store your Personal Information

Electronic:

  • All personal details are held on our client system Cliniko and no one has access to this other than our Associate Team. Sessions notes plus appointments, therapy agreements, new client information and any letters/reports are also held on Cliniko.
  • We also use Mailchimp to contact those who have given consent to be kept on our mailing list. Your name and email address are saved here and you can unsubscribe from the mailing list at any time. Your data is then deleted from the Mailchimp system.

 

How we may Process/Share/Keep your Personal Information

  • Your situation, presenting difficulties or background may be discussed with a supervisor in line with the Health and Care Professions Council (HCPC) and Royal College of Occupational Therapists (RCOT) standards of practice and code of ethics and professional conduct, however your identity will remain anonymous. This is in order to assist you further and ensure the support you receive is of most benefit.
  • All personal data is considered confidential and will not be passed to any other third party other than where you express consent to disclosure, in an emergency to protect someone’s health and wellbeing, or to comply with law or to the courts for administration of justice.
  • Wellbeing Therapy Solutions holds your details and clinical records for the duration of the therapeutic support and for 7 years post completion.
  • To ensure we provide the best possible therapeutic service, we must hold your data to comply with legal obligations including establishment and defence of legal rights, and the prevention of crime.

In line with GDPR terms, you have the following rights:

  • To be informed what information we hold
  • To see the information we hold about you
  • To rectify any inaccurate or incomplete personal information
  • To withdraw consent to Wellbeing Therapy Solutions using your personal information
